Referral Process

Referral by Health Provider: A health provider can fax a referral, or informally, refer a patient for services. Fax: 844-405-7247

Confirmation of Referral: The referring physician will receive a confirmation of receipt of the referral.

Booking an Appointment: After the referral is received, the client can book an appointment, or choose to first schedule a 15-minute free consultation through my website.

Informed Consent and Consultation with Referring Physician: Once treatment is underway and after discussing the informed consent process, if the client is interested in collaborative care and signs a consent form to allow consultation with the referring physician, the psychologist will contact the referring doctor to discuss the case.
